• Will receive $10,000.00 coverage after providing identification, address of insured, death certificate, printed goals accomplished.

• First, second, third months have – 20 new (and active) monthly member registrations, 50 sales with NG Rewards.
• Fourth month and forward – 5 new monthly registrations, 100 sales with NG Rewards.
• Employee has to complete three months prior to death.

• Goals 100% accomplished – $10,000.00 coverage.
• Goals 50% accomplished – $ 5,000.00 coverage
• Goals 25% accomplished – $ 2,500.00 coverage
